Before using this form you need to have registered.  This means you will have paid your deposit of $35 and received your confirmed registration number.  If you have yet received a number please ask via the Register page.

Sunday Night dinner is an extra $40 a head to attend.  The meal payment can be made now or at any time before the conference.  It is preferred that it be received a week prior to the conference, and must be made no later than at registration check in.
